Info for New Patients

The forms are available in three different file formats.

  • MSWord Form: These files are intedent to be filled out in MSWord and then printed.
  • MSWord Paper: These files are intendent to be printed from MSWord and filled out by hand.
  • PDF: These files are intendent to be printed from the Adobe Acrobat Reader and filled out by hand.


    • If you fill out the required forms before your first appointment, you will be able to send much more time talking to Dr. Santora and much less time filling out paper work.

    Warning: Completed forms should never be sent via email because they contain personal, confidential, information. Standard email is not a secure way to transmit such information.
